PROCEDURE
Read the
guidelines
of the Benevolence Team and decide if you think you may be eligible for
financial assistance from the Team.
Go to the
application and fill it out completely. The Team will not be able to
review your application in a timely fashion if information is missing. If you
have any questions on how to fill it out, please email the Team at

When you
have finished filling out the application, fax a copy to the Benevolence Team at
707-795-7017 and notify the Benevolence Team at
plwpbene@yahoo.com Send the original to the Benevolence Team via “snail
mail.” Please remember to keep a hard copy for your own records.

The
Benevolence Team will send you a notice confirming the receipt of the
application. The Benevolence Team will do everything in its power to make its
decisions in a timely fashion .In order to let you know the team’s decision as
soon as possible, you will receive notification of the team’s decision via
email. Unless you request otherwise, the email notification will be followed by
a hard copy sent via the US Postal Service for documentation.
Regardless
of whether or not the Benevolence Team accepts and approves your application for
assistance, there is no guarantee that whatever help is forthcoming, if it is,
will be effective or as effective as possible in solving the financial
difficulty presented in the application.
